We are seeking a highly skilled Foster Care Coordinator to join our team at UnitedHealthcare. As a Foster Care Coordinator, you will play a critical role in ensuring the health and well-being of children and youth in foster care.
Key Responsibilities- Assess and develop individualized care plans for children and youth in foster care, taking into account their unique needs and circumstances.
- Coordinate and monitor referrals for psychosocial, behavioral, and medical services to ensure timely and effective support.
- Act as a single point of contact for families and caregivers, providing education and support to help them navigate the foster care system.
- Advocate for children and youth to ensure their needs and choices are represented and supported by the healthcare team.
- Provide trauma-informed education and support to families and caregivers, helping them to better understand and address the needs of children and youth in foster care.
- Master's degree in social work, nursing, psychology, or related human services area.
- 2+ years of experience working in a community health setting, with a focus on children and youth in foster care.
- Ability to travel to visit Medicaid members in foster homes and other settings.
- Licensed Social Worker or clinical degree.
- Background in managing populations with complex medical or behavioral needs.
- Experience with electronic charting and arranging community resources.
