**Job Summary:**

We are seeking a highly skilled and compassionate Home Health Care Provider to join our team at Staten Island Care Center.

**About the Role:**

  • This is a full-time position that requires working with residents in their homes, providing essential care and support.
  • The successful candidate will be responsible for escorting residents to and from clinic appointments, ensuring they receive proper care and attention during these visits.

**Responsibilities:

Our ideal candidate will possess excellent communication skills, with the ability to effectively convey information about a resident's condition.

  • Assist residents with pre-appointment needs, such as preparing them for their outings.
  • Monitor residents to prevent falls and ensure a safe environment.
  • Report any incidents or accidents promptly to the Clinical Coordinator or ADNS.

**Requirements:

  • Proven ability to perform all tasks and duties of a Home Health Aide.
  • Strong command of the English language (spoken, written, and reading comprehension).
  • Expertise in providing age-appropriate care to residents.
  • Able to make independent decisions when needed and work well with staff, residents, families, and external partners.
