About the Role

We are seeking a skilled and compassionate MDS Nurse to join our team at Solaris HealthCare Apopka. As a key member of our interdisciplinary team, you will play a vital role in providing top-quality care to our residents.

Responsibilities
  • Conduct thorough and accurate MDS assessments to ensure our residents receive the best possible care.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident.
  • Provide education and training to non-licensed staff on the assessment process and care planning.
  • Assist with month-end duties to ensure accurate and timely completion of assessments and care plans.
Requirements
  • Graduate from an approved school of nursing with a current, unencumbered license to practice nursing in Florida.
  • Minimum of two years of RAI and Care Plan experience, preferably in a Skilled Nursing Facility (SNF).
  • Knowledge of MDS 3.0, including Assessments, CAA's, and Care Plans.
  • Ability to sit, stand, bend, twist, lift, and move intermittently throughout the day.
  • Able to see and hear adequately to respond and interact with other staff and families.
