We are seeking a skilled and compassionate MDS Nurse to join our team at Solaris HealthCare Apopka. As a key member of our interdisciplinary team, you will play a vital role in providing top-quality care to our residents.
Responsibilities- Conduct thorough and accurate MDS assessments to ensure our residents receive the best possible care.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident.
- Provide education and training to non-licensed staff on the assessment process and care planning.
- Assist with month-end duties to ensure accurate and timely completion of assessments and care plans.
- Graduate from an approved school of nursing with a current, unencumbered license to practice nursing in Florida.
- Minimum of two years of RAI and Care Plan experience, preferably in a Skilled Nursing Facility (SNF).
- Knowledge of MDS 3.0, including Assessments, CAA's, and Care Plans.
- Ability to sit, stand, bend, twist, lift, and move intermittently throughout the day.
- Able to see and hear adequately to respond and interact with other staff and families.
