Position Overview

As a Home Health Physical Therapist (PT), you will play a crucial role in our team. In this position, you will provide direct therapeutic care to patients while collaborating with an interdisciplinary team to ensure an exceptional care experience for both patients and their families. Key Responsibilities
- Deliver physical therapy services to patients based on a physician's prescribed care plan.
- Initiate and oversee physical therapy programs, instructing other staff and family members as necessary.
- Prepare and submit clinical documentation and progress reports.
- Conduct in-service training sessions for nursing staff as required.
- Engage in discharge planning activities.
- Supervise Physical Therapy Assistants in accordance with policies and state regulations.
- Ensure adherence to all applicable state and federal legal and regulatory standards.
- Perform additional duties as assigned.
- Graduate from a CAPTE-approved physical therapist education program with valid state licensure.
- A minimum of 2 years of experience in adult physical therapy.
- Experience in home health care is preferred.
- Dedication to clinical excellence and meticulous documentation.

Compensation Range
USD $90,000 - $110,000.00 / Year
