We are seeking a skilled Social Worker PRN to join our team at St. Mark's Hospital. As a Social Worker PRN, you will play a vital role in providing high-quality patient care and support to our patients and their families.
Key Responsibilities- Evaluate the psychosocial needs of patients and develop effective discharge plans to ensure a smooth transition to post-acute care.
- Act as a liaison between the hospital and external resources to coordinate patient care and services.
- Provide crisis intervention and support to patients and their families.
- Collaborate with the healthcare team to identify and address barriers to patient throughput.
- Master's degree in Social Work.
- Current State of Utah Certified Social Worker (CSW) license required; Licensed Clinical Social Worker (LCSW) license preferred.
- 2 or more years of clinical experience preferred.
