Social Services Coordinator

**Key Responsibilities:**
* Provide comprehensive intake assessments and develop comprehensive goals of care utilizing interdisciplinary team members, patients, family members, and/or caregivers.
* Present completed goals of care to patients and/or designees for acceptance of service.
* Communicate with service providers and city/state agencies.
* Document all activities related to the efficient administration and maintenance of patients in the home.
* Interview patients, family members, and caregivers to evaluate medical, psychosocial, financial, and environmental needs.
* Formulate accurate assessments, intervention plans, and assist with enrollment in eligible programs.
* Assist patients, family members, and caregivers in resolving psychological barriers that impede optimum social and health adjustment.
* Maximize patients', family members', and caregivers' ability to comply with medical recommendations and goals of care.
**Requirements:**
* Master's degree in social work.
* 1-2 years' experience.
* LGSW in proper jurisdiction.
* American Heart Association (AHA) or Red Cross CPR certification.
* Home health care experience preferred.
