We are seeking a compassionate and skilled Registered Nurse to join our Hospice team at AdvisaCare. As a Hospice RN, you will play a vital role in providing emotional support and care to patients and their families during their final days.
Key Responsibilities- Conduct initial and ongoing assessments of patients' physical, functional, psychosocial, and environmental needs.
- Implement individualized plans of care and make revisions as needed to ensure the best possible outcomes for patients.
- Comply with AdvisaCare Hospice policies and procedures, as well as Medicare regulations.
- Consult with and educate patients and their families on disease processes, self-care techniques, end-of-life care, and ethical concerns.
- Supervise paraprofessionals providing services to patients and prepare clinical and progress notes.
- Participate in Interdisciplinary Group (IDG) conferences and contribute to the Hospice performance improvement program.
- Maintain high levels of customer satisfaction with clients, families, and caregivers.
- Communicate effectively using current processes and technology within the organization.
- Current Michigan license in good standing.
- Minimum 1 year of hospice experience.
- Empathetic approach to patients and caregivers.
- Ability to adapt to patients' status and needs on a full-time on-call basis.
