Role Responsibilities and QualificationsThe Case Management RN is integral in delivering patient-centered care by overseeing the care plan throughout the patient's stay. Responsibilities include:
- Identifying patients at risk for adverse outcomes during care transitions.
- Conducting thorough assessments of patients' psychosocial, medical, and discharge needs.
- Collaborating with the healthcare team to develop and implement effective discharge plans.
- Facilitating communication between patients, families, and healthcare providers.
- Documenting care coordination efforts and ensuring compliance with established protocols.
- Engaging in performance improvement initiatives to enhance patient outcomes.
Qualifications:
- Associate Degree in Nursing or Nursing Diploma required.
- Bachelor's Degree in Nursing preferred.
- Minimum of 2 years of case management experience or 3 years in clinical nursing.
- Familiarity with InterQual criteria.
