Job Title: Intake Coordinator

Prologue Inc is seeking a highly skilled Intake Coordinator to join our team. The successful candidate will be responsible for screening individuals for admission into our Psychiatric Rehabilitation Program (PRP) and Residential Rehabilitation Program (RRP).

Key Responsibilities:

  • Screen individuals for admission into PRP and RRP
  • Obtain consent for services, medical records, and other necessary documentation
  • Obtain necessary billing authorizations and determine any fees for services
  • Develop an initial plan of action and link individuals with their respective rehabilitation team
  • Work with the local Department of Human Services to secure entitlements such as Medicaid, Public Assistance to Adults, and Supplemental Nutritional Assistance Program

Requirements:

  • Bachelor's degree with at least one year of experience in mental health/psychiatric rehabilitation/related human services
  • Ability to work independently and with a team as necessary
  • Good driving record
  • Ability to respect individual choice in the recovery process and a commitment to recovery through the psychiatric rehabilitation process
