Job Overview

The Clinical Pharmacy Specialist in Organ Transplantation plays a critical role at the University of Chicago Medical Center. This position is responsible for providing comprehensive pharmacy services to adult and pediatric patients undergoing solid organ transplantation.

The ideal candidate will possess strong clinical knowledge and expertise in traditional therapeutics and patient-centered care, as well as significant knowledge and expertise in advanced pharmacy practice, medication therapy management, and evidence-based medicine.

  • Key Responsibilities:
  • Providing decentralized clinical services to adult and pediatric recipients of kidney, pancreas, liver, heart, and lung transplant
  • Completing pre-transplant pharmacy evaluations
  • Managing transitions of care
  • Providing medication-related education to patients and healthcare team members
  • Precepting pharmacy students and residents
  • Maintaining programmatic infrastructure, including protocol development, outcomes review, and quality assurance

Requirements:

  • Holds a Doctor of Pharmacy from an ACPE-accredited School of Pharmacy
  • Has completed a PGY1 residency
  • Has completed a PGY2 Solid Organ Transplant residency (strongly preferred)
  • Knowledge of contemporary hospital/clinical practice and service
  • Strong written and verbal communication skills
  • Ability to interact constructively with supervisors, peers, and support personnel
