We are seeking a dedicated and compassionate Home Health Aide to join our team at CenterWell Home Health. As a Home Health Aide, you will play a vital role in providing personalized care to patients in the comfort of their own homes.
Key Responsibilities- Provide direct patient care under the supervision of a Registered Nurse and in accordance with the Aide Plan of Care.
- Assist patients with self-administered medications, ensuring accurate dosing and observation.
- Take accurate temperature, pulse, and blood pressure measurements, and report any abnormal results to the supervisor.
- Help patients maintain good personal hygiene, including bathing, grooming, and skin care.
- Assist with feeding patients, communicating basic nutrition principles, and observing and recording food and fluid intake.
- Provide necessary skills to safely assist patients with mobility, exercises, positioning, transfers, and ambulation, as per the Plan of Care and CenterWell Home Health policy.
- Document interactions with patients, caregivers, doctors, and other staff members, ensuring continuity of care.
- Practice acceptable infection control principles and provide a clean, safe, and comfortable environment.
- High school diploma or equivalent.
- Completion of a Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
- Must meet applicable state certification requirements.
- A valid driver's license, auto insurance, and reliable transportation are required.
- Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency.
- At least one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.
