Job Summary

We are seeking an experienced MDS Nurse to join our team at DTN Staffing. As a key member of our healthcare team, you will be responsible for assessing patients, developing care plans, and monitoring their health and well-being.

Key Responsibilities:

  • Conduct thorough patient assessments to collect information about their health, including behavioral, psychological, clinical, nutritional, and physical needs.
  • Develop and implement individualized care plans that meet the patient's needs and goals.
  • Monitor patients from admission through their stay to ensure their health and well-being.
  • Collaborate with staff, patients, and their families to address concerns and provide excellent patient care.
  • Collect and submit data to ensure compliance with government guidelines.
  • Help patients enroll in federally-funded health benefits and serve as a liaison between facility management and insurance providers.

Requirements:

  • Several years of experience in patient assessments and care planning.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of government guidelines and regulations.
