About the Role

We are seeking a skilled Occupational Therapist to join our team and work with seniors in a dynamic care coordination program. As an OT, you will provide occupational therapy and wellness services, conduct assessments and treatment planning, and support care coordination efforts.

Responsibilities

  • Provide occupational therapy services to older adults, including assessments, treatment planning, and therapeutic interventions
  • Support care coordination efforts by conducting wellness visits, communicating with patients' providers and caregivers, and assisting with telehealth visits
  • Maintain professional practices and ethical standards in accordance with company policies and clinical initiatives
  • Contribute to program development, quality improvement, and problem-solving
  • Optimize each patient's functional well-being and satisfaction

Requirements

To be successful in this role, you will need:

  • Completion of an accredited Occupational Therapy program
  • Current state licensure in OT
  • Excellent verbal and written communication skills
  • A results-oriented mindset with critical thinking skills
  • Tech-savvy skills and a willingness to learn
