Primary Care Clinician Specialist

As a member of our care team, you will have the opportunity to work closely with various healthcare professionals to deliver comprehensive patient care. Our integrated care teams include physicians, nurses, behavioral health specialists, and more.
Key Responsibilities- Evaluate and treat patients according to established standards of care.
- Develop and implement personalized care plans that address patients' unique needs and goals.
- Collaborate with the care team to ensure seamless coordination of patient services, including specialty referrals, hospital coordination, and home health care.
- Communicate effectively with patients and their families, addressing concerns and providing education on disease management and prevention.
- Stay up-to-date on the latest clinical guidelines and best practices in primary care.
- Graduate of accredited MD or DO program from an accredited university.
- Current and unrestricted medical license or willingness to obtain a medical license in state of practice; eligible and willing to obtain licenses in other states in the region of assignment, as required.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team-based environment.
- Proficiency in electronic medical records (EMRs).
