Job Summary

We are seeking a highly skilled and compassionate Family Specialist to join our team at the Institute for Community Living. As a Family Specialist, you will play a critical role in providing comprehensive mental health services to individuals with serious and persistent mental illness and their families.

Key Responsibilities
  • Provide counseling, treatment, and rehabilitation services to individuals and their families in the community, including homes, shelters, and streets.
  • Assume primary clinical management responsibility for a caseload of 10-15 adults with serious and persistent mental illness, including those with co-occurring substance abuse or other disorders.
  • Lead the integration of family goals and services with the tasks of all team members and provide family psycho-education individually and in groups.
  • Assist consumers in attaining or maintaining housing, employment, functional social relationships, and relief from psychiatric symptoms and side effects through medication management support.
  • Engage in assertive outreach and provision of mental health services in the community, with up to 80% of the weekly work schedule involving fieldwork.
  • Provide 24-hour responsibility for individuals enrolled in the ACT Team caseload.
  • Provide services in a culturally relevant and competent manner.
  • Maintain accurate and up-to-date Case Records, including progress notes, service plan and service plan reviews, assessments, and contact logs.
  • Provide support and psychoeducation to family members, friends, and other collateral individuals.
  • Provide referrals and engagement support for services not directly provided by the ACT Team, including medical services, psychosocial clubhouses, self-help groups, vocational training, and placement.
  • Provide accurate and timely reports to other providers and agencies, within federal, state, and city regulatory and agency confidentiality guidelines.
  • Participate in individual and group supervision, treatment team meetings, case conferences, in-service training, and other staff development activities as assigned.
Requirements

We are seeking a Master's Degree in a discipline recognized by the New York State Office of Mental Health as qualifying a staff person to be counted as professional clinical staff, such as Social Work, Psychology, Occupational Therapy, Rehabilitation Counseling, Pastoral Counseling, Creative Arts Therapy, or Therapeutic Recreation. A professional license in the field issued by the New York State Education Department is strongly preferred. Additionally, specialized experience or training of at least one year in family assessment and intervention, psychoeducation, and/or other family support services, including cognitive-behavioral strategies, is required.
