You will be part of a dedicated and collaborative team that values effective communication and teamwork.
Key Responsibilities
- Complete the MDS, CAA's, and care plans within mandated timelines.
- Coordinate the scheduling of the RAI process with the interdisciplinary team.
- Conduct resident assessments through physical evaluations, interviews, and chart reviews.
- Engage with caregivers, including physicians, nursing staff, social services, therapy, dietary, and activities personnel to discuss resident care needs.
- Review information from hospitals, consultants, and external agencies to inform assessment and care planning.
- Identify and initiate significant changes in MDS as necessary.
- Prepare for and conduct PPS meetings, ensuring MDS assessments align with Medicare schedules, while maintaining oversight of the PPS board for monitoring Medicare days and RUGs utilization in the absence of the Care Management Coordinator.
- Stay updated with the requirements set forth by the American Association of Nursing Assessment Coordinators (AANAC).
Qualifications
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure.
- AANC certification is preferred; RAC-CT certification is a plus.
- Strong understanding of the Resident Assessment Instrument (RAI) process and the principles of the Prospective Payment System (PPS) is highly desirable.
- Previous experience as an MDS Nurse is essential.
