Job Summary

We are seeking a dedicated and compassionate Direct Support Professional to join our team at Hearts & Hands of Care, Inc. As a Direct Support Professional, you will play a vital role in providing ongoing services to clients and individuals with needs in their community or in their home.

Key Responsibilities:

  • Provide high-quality care and support to clients, ensuring their safety and well-being at all times.
  • Develop and implement individualized Service Plans, working collaboratively with clients, families, and team members to achieve measurable objectives.
  • Document daily activities, progress, and challenges, maintaining accurate and comprehensive records.
  • Engage in friendly, professional relationships with clients, families, and colleagues, maintaining open communication and boundaries.
  • Stay up-to-date with knowledge of services and supports available in the community, both within and outside of our agency.
  • Coordinate transportation for clients as needed.
  • Interact professionally with other stakeholders, including families, healthcare providers, and community resources.

Requirements:

  • Must be at least 18 years of age.
  • Meet all requirements for the position as set out in 7 AAC 125.010 – 7 AAC 125.199.
  • Pass a Criminal Background Check.
  • Have a valid First Aid card issued by the American Red Cross, American Heart Association, or other agency approved by The Department of Seniors and Disability Services.
  • Attend Mandt training.
  • Have confirmed service in the industry with advance training in behavioral, medical, or caregiver services (preferred).
