We are seeking an experienced Physical Therapist to join our team at Floral Long Term Care.
Job Summary:
The Physical Therapist will be responsible for assessing patients who require rehabilitation services, providing direct patient care, and establishing treatment programs with specific goals. The ideal candidate will have a valid professional license, registration, and experience in long-term care and geriatric care.
Key Responsibilities:
- Assess patients who require rehabilitation services
- Provide direct patient care, including evaluations, progress notes, and discharge documentation
- Establish treatment programs with specific goals
- Supervise Physical Therapy Assistants
- Collaborate with other disciplines to provide best patient-centered treatment and care
- Report to work on time and adhere to scheduled hours
Requirements:
- Graduated from an accredited Physical Therapy college or university
- Valid professional license and registration
- Experience in long-term care and geriatric care
- Well-versed in documentation writing and compliance with confidentiality, HIPAA, and Federal Residents Rights
