Key Responsibilities:
  • Deliver comprehensive nursing visits and case management to home health patients, ensuring their needs are met and their health is optimized.
  • Collaborate with physicians to develop and implement individualized treatment plans, providing hands-on care under the supervision of a Clinical Manager.
  • Coordinate care with healthcare professionals, including Home Health Aides and Personal Care Workers, to ensure seamless patient care.
  • Implement patient care plans, supervise Home Health Aides and Personal Care Workers, and communicate with other healthcare disciplines to ensure effective patient outcomes.
  • Discharge patients, prepare clinical documentation, and communicate with other healthcare professionals to ensure a smooth transition of care.
Requirements:
  • Diploma, Associate, or Bachelor's Degree in Nursing.
  • Minimum one year of nursing experience preferred.
  • Home Health experience is advantageous.
  • Current and unrestricted Registered Nurse licensure and CPR certification.
Work Schedule:

Full-time, 40 hours per week.

Compensation:

$80,000 - $110,000 annually.
