Job Summary

We are seeking a licensed practical nurse (LPN) to join our team as a Home Health Nurse - Behavioral Health Visits FT. In this role, you will work with patients who require behavioral health support, providing skilled nursing care and interventions to help them achieve their goals.

Estimated Salary: $63,000 - $80,000 per year

This is a full-time position that requires working Saturdays and Sundays, plus one day during the week. The ideal candidate will have experience in an acute care or equivalent setting and be certified in CPR.

Responsibilities
  • Provide skilled nursing care and interventions to patients requiring behavioral health support
  • Work collaboratively with the interdisciplinary team to develop and implement patient care plans
  • Maintain accurate and up-to-date records of patient progress and outcomes
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions
Requirements
  • Licensed Practical Nurse (LPN) license in good standing
  • 1 year of experience in an acute care or equivalent setting
  • CPR certification with American Heart Association or American Red Cross
  • Positive attitude and strong communication skills
