Nurse Case Manager

Mass General Brigham is seeking a skilled Nurse Case Manager to support complex patient discharges and optimize transitional care planning.
The successful candidate will work closely with case managers, internal and external resources, and Case Management Leadership to identify and optimize complex discharge planning.
This role requires strong assessment and problem-solving skills, as well as the ability to work independently with minimal supervision.
Key Responsibilities- Research and identify resources to support complex discharge planning
- Collaborate with case managers and internal/external resources to optimize discharge planning
- Provide day-to-day support and guidance to case managers on complex case management
- Identify and mitigate discharge barriers to ensure safe care transitions
- Develop and implement best-practices and new practices related to discharge planning
- RN with current license to practice in Massachusetts
- Bachelor's degree required; master's degree preferred
- Case Manager certificate preferred; years of inpatient case management experience with advanced knowledge around standards of practice and regulatory and payer compliance required
- Extensive experience and knowledge in discharge planning and post-acute resources required
