We are seeking a skilled Case Manager - Social Work Specialist to join our team at Novant Health. Key Responsibilities- Collaborate with physicians and other healthcare professionals to assess, plan, implement, and evaluate discharge plans
- Facilitate coordination of care and serve as a resource for discharge planning and care coordination for psychosocially complex cases
- Maintain current knowledge of available community resources and post-acute care options
- Work closely with RN Case Managers to provide guidance on medically complex cases
- Accept responsibility, authority, and accountability for management of care, in accordance with current policies and procedures, mission, vision, and values of Novant Health
- Graduate degree in Social Work from a school accredited by the Council on Social Work Education
- Two or more years of professional social work experience or two or more years of healthcare experience, preferred
- Case management or related experience, preferred
- NC state-approved curricula for restrictive interventions (facility-specific) within 3 months of hire for Emergency Departments and Behavioral Health units, required
