We are seeking a skilled Care Navigator to join our team at Cahaba Medical Care Foundation. This role will involve working closely with patients to connect them with necessary resources and services.

Estimated Salary: $45,000 - $65,000 per year, based on experience and qualifications.

About the Role:

  • Administration of Sliding Fee Discount Program: The successful candidate will work with the billing office to administer the sliding fee discount program, ensuring that eligible patients receive financial assistance.
  • Patient Care Planning: As a Care Navigator, you will assist providers in creating individualized care plans for patients, addressing social determinants of health and chronic disease management.
  • Community Resource Management: You will maintain an active list of community resources and help patients access these services, including charity care programs, transportation assistance, and medication support.
  • Crisis Intervention and Advocacy: In this role, you will provide crisis intervention services and advocate for patients with various agencies, promoting their well-being and safety.

Requirements:

  • Bachelor's Degree in Social Work or related field
  • Social Work License (registered with the Alabama Board of Social Work Examiners)
  • Excellent organizational, interpersonal, and communication skills
  • Strong computer and internet research skills, with proficiency in Google Apps, Microsoft Office, and Adobe products
  • Ability to work collaboratively with internal staff and external vendors
