Job Title: Director of Social Work and Admissions

At Apple Rehab Colchester, we are seeking a highly skilled and compassionate Director of Social Work and Admissions to join our team. As a key member of our leadership team, you will be responsible for implementing and documenting plans of care related to residents' social and emotional needs, collaborating with nursing, MDS, and rehabilitation departments to facilitate discharge plans, and maintaining updated resources for residents and families.

Responsibilities:
  • Implement and document plans of care related to residents' social and emotional needs
  • Collaborate with nursing, MDS, and rehabilitation departments to facilitate discharge plans
  • Maintain updated resources for residents and families
  • Inform and educate staff, residents, families, and responsible parties of residents' personal and property rights, as well as their rights related to health services
  • Organize, control, and execute the admission process, including referral management, community and hospital sales, admitting new residents, and completing admission agreements in compliance with all regulations and guidelines
  • Coordinate facility-specific and company activities, including attending external community marketing events and arranging tours of the facility
Requirements:
  • Bachelor's or Master's degree from an accredited program in Social Work or a related field
  • Previous experience in a long-term care facility and knowledge of current Requirements of Participation regulations
  • Attention to detail, good follow-through skills, and ability to prioritize multiple tasks
  • Innovative and able to find creative ways to serve the variety of resident needs and maintain a customer-focused approach
  • Valid driver's license and a vehicle
  • Ability to communicate with residents, staff, and family members in a professional manner
