ChenMed is seeking a highly skilled and compassionate Primary Care Physician to join our team. As a Primary Care Physician, you will play a key role in providing high-quality patient care and contributing to the growth and success of our organization.
Key Responsibilities- Provide direct patient care and assessments in an ambulatory healthcare setting, with occasional visits to acute care, nursing homes, skilled nursing facilities, and home settings.
- Conduct geriatric assessments, medical histories, physical exams, diagnoses, and treatments, as well as develop and implement plans of care.
- Collaborate with other healthcare professionals to ensure high-quality patient care and outcomes.
- Participate in clinical rounds and conferences, and maintain accurate and detailed documentation of patient progress.
- Consult with managers and medical directors to ensure compliance with guidelines and participate in risk and quality management programs.
- Current licensure in the state of practice is required.
- Certification in Basic Cardiac Life Support is required.
- Board certification in Internal Medicine or Family Medicine is preferred, with the requirement to achieve Board certification within 2 years.
- A minimum of one year of clinical experience in geriatric, adult, or family practice is preferred.
- A current DEA number for schedule II-V controlled substances is required.
