Job Summary

CareConnect is seeking a compassionate and skilled Home Health Aide to provide exceptional care to our clients in the comfort of their own homes. As a Home Health Aide with CareConnect, you will play a vital role in ensuring the safety and well-being of our clients, while also providing emotional support and companionship.

Key Responsibilities
  • Personal Care: Assist clients with bathing, dressing, and grooming, ensuring their personal hygiene and comfort.
  • Mobility and Transfers: Safely assist clients with mobility and transfers, preventing falls and injuries.
  • Medication Administration: Administer medications as directed by healthcare professionals, ensuring accurate and timely administration.
  • Vital Sign Monitoring: Monitor clients' vital signs, reporting any changes or concerns to the appropriate medical personnel.
  • Emotional Support: Provide emotional support and companionship to clients and their families, promoting a sense of comfort and security.
  • Environmental Maintenance: Maintain a clean and safe environment for clients, ensuring their living space is free from hazards and clutter.
Requirements
  • Certified Personal Care Assistant (PCA) or Certified Home Health Aide (HHA) certification.
  • Understanding of HIPAA regulations and the importance of patient confidentiality.
  • Ability to cook simple meals and perform light housekeeping duties as needed.
  • Satisfactory completion of New York State Health Department-approved HHA Training Program or equivalency.
  • Demonstrated adequate literacy and simple arithmetic skills.
  • Must be at least 18 years old.
