Job Summary

We are seeking a dedicated and compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our team. As a PMHNP, you will be responsible for conducting comprehensive psychiatric evaluations and assessments of elderly patients, diagnosing and developing individualized treatment plans, and providing medication management and counseling services.

Responsibilities
  • Conduct thorough psychiatric evaluations and assessments of elderly patients to identify mental health needs.
  • Develop and implement individualized treatment plans to address patient needs, including medication management and counseling services.
  • Collaborate with interdisciplinary teams, including physicians, psychologists, and social workers, to ensure holistic patient care.
  • Provide education and support to patients and their families on mental health conditions, treatment options, and self-care strategies.
  • Maintain accurate and up-to-date medical records in compliance with organizational and regulatory standards.
  • Participate in clinical meetings, case conferences, and quality improvement initiatives to ensure high-quality patient care.
Requirements
  • Valid Nurse Practitioner license in the state of New York with specialization in Psychiatry (PMHNP).
  • Ability to work independently and effectively manage a caseload of patients.
  • Strong diagnostic skills and knowledge of psychiatric medications and treatment modalities.
  • Excellent interpersonal and communication skills to establish rapport with patients, families, and interdisciplinary team members.
  • Proficient in electronic health record (EHR) systems and basic computer skills.
  • Compassionate, patient-centered approach to care.
