Job Summary:

The successful candidate will be responsible for providing high-quality, personalized care to patients, developing and implementing effective treatment plans, and collaborating with our interdisciplinary team to achieve optimal patient outcomes.

Key Responsibilities:
  • Conduct thorough assessments and evaluations to identify patient needs and goals
  • Develop and implement individualized treatment plans using a variety of evidence-based techniques
  • Provide education and training to patients and their families on proper techniques and strategies for optimal recovery
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination
  • Document patient progress and outcomes in a timely and accurate manner
Requirements:
  • Bachelor's or Master's degree in Physical Therapy from an accredited institution
  • Current state licensure to practice as a Physical Therapist
  • Experience in a skilled nursing facility (SNF) setting is a plus
  • CPR certification
Physical Demands:

The successful candidate must be able to:

  • Stand and walk for extended periods
  • Lift and carry up to 25 pounds
  • Push and pull carts and transport residents
  • Meet the general health requirements set forth by the policies of this facility
