Job Title: Advanced Practice Provider - NP or PA

Catholic Health Physician Partners is seeking a skilled Advanced Practice Provider to join our team. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our physicians to deliver exceptional patient outcomes.

Job Summary:

As an Advanced Practice Provider, you will be responsible for providing comprehensive care to assigned patients, including medical interventions, monitoring laboratory and test results, and obtaining appropriate consultations as needed.

Responsibilities:
  • Provide comprehensive care to assigned patients along with attending physician
  • Providing medical interventions as indicated
  • Monitoring laboratory and test results
  • Obtaining appropriate consultations as needed
  • Assist with patient discharges/discharge planning
  • Monitoring patient progress in rehabilitation for safe discharge to home or other facilities
  • Demonstrating care that reflects an understanding of individual cultural and religious diversities and needs, along with the recognition of patient rights
  • Thoroughness of patient care
  • Medical and clinical knowledge especially in PM&R, clinical judgment and technical skills, utilize current "best practices"
  • Ability to work with others, establish rapport with patient and families
  • Maintain patient confidentiality and demonstrate ethical standards in treatment
  • Commitment to continuous personal development
  • Clarity and legibility of medical records - timely and completeness of documentation
  • Advocate for quality patient care
  • Assist patients dealing with system complexities
Requirements:
  • NY State RN license and Registered NYS Nurse Practitioner license, and adult, family or geriatric certification
  • BLS certification required, as is critical thinking abilities and superior interpersonal and technical skills
