Job Summary

Shanoan Springs Nursing and Rehab is seeking a compassionate and skilled LPN to join our team as a Licensed Practical Nurse. As a key member of our healthcare team, you will be responsible for providing high-quality nursing care to our residents, ensuring their comfort, dignity, and well-being.

Responsibilities
  • Supervise, provide, and coordinate nursing care in compliance with facility policies and procedures.
  • Assess residents' care needs and take appropriate action to ensure their comfort and well-being.
  • Embody person-centered care by promoting individual resident choice and controlled resident-preferred routines in all aspects of daily living.
  • Collaborate with the healthcare team to develop and implement individualized care plans.
  • Provide education and support to residents and their families on health-related topics.
  • Participate in quality improvement initiatives to enhance the quality of care and services provided.
Requirements
  • Current LPN license in good standing.
  • Supervisory experience preferred.
  • Ability to read, write, speak, and understand English.
  • Effective documentation and oral communication skills.
  • Ability to comprehend applicable Federal, State, and local regulations and facility policies, procedures, and programs.
  • Ability to direct, delegate, and follow-up on programs and care of the residents.
  • Meet all health requirements imposed by law.
  • Ability to use/demonstrate use of all nursing service equipment.
