Nurse Practitioner

Fenway Health is seeking a highly skilled and compassionate Registered Nurse to join our team. As a Registered Nurse at Fenway Health, you will provide exceptional patient care and work collaboratively with our healthcare team to deliver high-quality services.
Key Responsibilities- Collaborate with patients, families, and healthcare providers to develop and implement individualized care plans.
- Provide comprehensive nursing care, including assessment, diagnosis, planning, implementation, and evaluation of patient care.
- Conduct virtual, telephonic, or in-person nurse assessments and triage to assess the severity of patient health concerns.
- Implement direct and indirect nursing care consistent with evidence-based practices, hospital policies, and procedures.
- Coordinate preventative services, including education, preventative medications, vaccinations, testing, and referrals.
- Document patient information accurately and in a timely manner in the Electronic Health Record (EHR).
- Participate in daily operational activities necessary for a safe patient and team environment.
- Identify complexities within the healthcare system and participate in resolving barriers to effective, efficient, and fiscally responsible care delivery.
- Associate degree in nursing.
- Current Massachusetts Registered Nurse (RN) license.
- Current CPR certification.
- Effective interpersonal and communication skills.
- Experience working in an ethnically, culturally, and racially diverse environment.
- Authentic interest in LGBTQIA+ and community health.
- Bachelor's degree level or higher in nursing.
- Two (2) years of experience in ambulatory, hospital, or public health nursing.
- Experience with working within a complex healthcare system with multiple product lines.
- Board Certification (ACRN, AMB-BC, etc).
