Job Summary:

As a skilled Registered Nurse Home Health Case Manager, you will provide high-quality nursing care and case management to patients in their place of residence. This role requires a strong understanding of home health care principles and the ability to work independently with minimal supervision.

Responsibilities:

Provide skilled nursing care under physician orders

Conduct thorough nursing assessments and develop individualized plans of care

Collaborate with patients' physicians to ensure quality care and coordinate services

Manage and coordinate home care needs for 25-30 patients, utilizing all available agency and community resources

Maintain accurate and timely documentation, including point of care documentation and admission/recertification/resumption of care assessments

Requirements:

Registered Nurse licensed with the State of Minnesota

At least one year of Medical/Surgical or related nursing experience

Home Health Care experience preferred

Work Environment:

Requires daily travel by automobile in all weather conditions

Occasional exposure to unclean or unsanitary conditions during home visits

Physical Demands:

Sitting, driving, standing, walking, repeated bending, squatting, stooping, or similar strenuous duties

Lifting up to 50 pounds occasionally

Competitive salary range: $83,000.00 - $93,000.00 per year
