Job Summary

We are looking for a dedicated Physical Therapist to provide high-quality patient care in the comfort of their own homes. As a member of our team, you will be responsible for assessing and evaluating patient care needs, developing and implementing treatment plans, and collaborating with healthcare professionals to ensure optimal patient outcomes.

Responsibilities
  • Evaluate patient functional status and physical therapy needs, assisting the physician in developing the therapy plan of care.
  • Observe and report patient response to treatment and any changes in condition to the supervising nurse and/or physician.
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
  • Instruct and inform participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
Requirements
  • Current Physical Therapy licensure in the state of New Jersey.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
