About the Role

We're seeking a skilled and compassionate Physical Therapist to join our team in Bloomington, IN and surrounding areas. As a Geriatric Care Specialist, you'll provide evidence-based therapy to older adults in their homes, helping them to maintain their independence and improve their quality of life.

What You'll Do
  • Deliver proactive and evidence-based therapy to older adults in their homes
  • Develop and implement personalized care plans to meet the unique needs of each patient
  • Collaborate with our interdisciplinary team to ensure seamless care coordination

What We're Looking For
  • A valid Physical Therapy license in the state(s) of practice, or eligibility to apply
