We are seeking a skilled Nurse Practitioner or Physician Assistant to join our Home-Based Medical Care team in Muskegon, MI. As a member of our interdisciplinary team, you will provide high-quality, patient-centered care to complex, chronic patients in the comfort of their own homes.
Key Responsibilities:- Deliver comprehensive, coordinated care to patients with multiple chronic conditions
- Collaborate with patients, caregivers, and healthcare providers to develop and implement personalized care plans
- Conduct regular home visits to assess patient needs and provide ongoing care
- Prescribe medications and treatments as needed to manage patient conditions
- Participate in ongoing education and training to stay current on best practices and research in home-based medical care
- Current NP/PA license
- Completion of Nurse Practitioner or Physician Assistant accredited program
- Access to reliable transportation
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
- Knowledge of long-term care, community resources, and cost-effective alternatives
- Knowledge of CPT, ICD-10, and HCPC codes
