Job Summary

We are seeking a skilled Geriatric Medicine Physician to join our team at Sage Health. As a key member of our care team, you will be responsible for evaluating and treating patients in accordance with standards of care.

Key Responsibilities
  • Evaluate and treat patients in accordance with standards of care
  • Assist in the coordination of patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care
  • Participate in daily huddles with the care team to discuss patient care and medical expertise
  • Work with the Lead Physician and Center Administrator to set a tone of cooperation in practice
  • Complete all medical record documentation daily, working with a quality-based coder to optimize comprehensive documentation
  • Follow policy and protocol defined by Clinical Leadership
  • Meet with the Lead Physician to discuss quality of care, review of outcome data, and medical records issues
Requirements
  • Graduate of an accredited MD or DO program
  • Current and unrestricted medical license or willingness to obtain a medical license in the state of practice
  • Board Certification or Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine preferred
  • Active and unrestricted DEA license
  • Medicare Provider Number
  • Medicaid Provider Number
  • Excellent interpersonal and communication skills
  • Fully engaged in the concept of team-based care model
  • Ability to learn and adapt to practice in a value-based care setting
  • Superior patient/customer service skills
Preferred Qualifications
  • Knowledge of HEDIS/STARS
  • Knowledge of CAHPS and HOS
  • Experience working in medically underserved/culturally diverse communities
  • Experience working with an electronic health record
  • Experience working in Medicare Advantage Global Risk Models
