Key Responsibilities- Participate in the medical evaluation of patients admitted to the Skilled Nursing Facility.
- Evaluate and document the medical necessity for specific levels of care, facilitating transfers if necessary.
- Provide advice and information to staff and referring physicians on medical interventions consistent with the medical plan of care.
- Consult with attending physicians and referral sources to ensure comprehensive and efficient medical care for patients.
- Communicate with patients and families regarding their medical condition and plan of care.
- Participate in the review and development of practice protocols within the facility.
- Inform primary physicians and referral sources of the outcomes of Nurse Practitioner consultations and evaluations.
- Document interactions between nursing staff, referral sources, patients, and families.
- Maintain regular contact with assigned referral sources.
- Complete, maintain, and submit accurate clinical notes regarding patients' conditions and care.
- Utilize electronic medical records to ensure quality care, accuracy, and effective communication with the Skilled Nursing Facility.
- Provide education and information to the SNF care team and/or medical community as needed.
- Participate in resolving interpersonal conflicts and clinical or ethical concerns.
- Engage in research activities or special projects as assigned.
- Participate in the call schedule with other members of the call team (compensated separately).
- Perform other duties as assigned.
- Minimum of two (2) years of professional experience in healthcare.
- Experience in a skilled nursing facility preferred.
- Experience in acute care in an institutional setting preferred.
- Nurse Practitioner or Physician Assistant licensure is required.
- Possess and maintain current CPR Certification.
- Current Massachusetts licensure as a Nurse Practitioner or Physician Assistant.
- Valid driver's license.
May be exposed to infections and contagious diseases. Contact with patients under a variety of circumstances. Subject to varying and unpredictable situations. Handles emergency or crisis situations. Travel required.
OSHA exposure category: Category I (tasks involve exposure to blood, body fluids, tissues).
