About the Role

We are seeking a skilled Nurse Practitioner to join our team as a Telemetry Care Specialist at SSM Health. This is an exciting opportunity to provide high-quality patient care and work collaboratively with our healthcare professionals.

Job Summary

The successful candidate will be responsible for delivering direct nursing care to patients in the telemetry department, working closely with our multidisciplinary team to ensure seamless patient outcomes. This role requires excellent communication and leadership skills, with a focus on providing compassionate and culturally sensitive care.

Key Responsibilities
  • Patient Assessment and Care Planning: Develop and implement individualized care plans that address the unique needs of each patient, taking into account their physical, functional, cultural, social, economic, developmental, spiritual, vocational, and leisure dimensions.
  • Clinical Practice and Decision Making: Utilize evidence-based practices to make informed decisions about patient care, ensuring that interventions are safe, effective, and aligned with best practices.
  • Collaboration and Communication: Foster strong relationships with patients, families, and healthcare teams to ensure effective communication and coordination of care.
  • Educational and Advocacy Roles: Educate patients and families about measures that promote, maintain, and restore health or promote comfort, advocating on their behalf to ensure optimal patient outcomes.
Requirements

To be successful in this role, you will need:

  • A graduate degree in Nursing or related field.
  • Current RN licensure in the state where the position is located.
  • Basic Life Support (BLS) certification from the American Heart Association (AHA).
