Job Description

We are seeking a highly skilled Registered Nurse to lead our home care team, delivering exceptional patient care and building strong relationships with patients, families, and healthcare professionals.

Responsibilities
  • Patient Care Coordination: Develop and implement customized care plans, ensuring patients receive coordinated, patient-centered care.
  • Clinical Leadership: Supervise LPNs and HHAs, providing expert guidance and mentorship to optimize patient outcomes.
  • Educational Programs: Design and deliver comprehensive educational programs for patients and their caregivers, empowering them to manage their conditions effectively.
  • Team Collaboration: Foster open communication and collaboration among healthcare teams to ensure seamless care coordination.
Qualifications
  • Education: One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical.
  • Certifications: Current RN license, specific to the state(s) you are assigned to work, and current CPR certification.
  • Licenses: Valid driver's license, reliable transportation, and liability insurance.
Salary Range

$101,000-110,000 annually, commensurate with experience and qualifications.
