Pediatric Complex Care RN Case Manager

This is an exciting opportunity within the Pediatric Complex Care Collaborative (PC3) program at Johns Hopkins Children's Center. The PC3 program aims to improve the lives of our most complex patients and their families through collaboration and coordination across the care continuum, with an emphasis on keeping the patient at home whenever possible.
Key Responsibilities- Work with the multi-disciplinary team to coordinate and streamline patient/family experience, following complex cases across hospital settings and the care continuum.
- Exclusively serve the PC3 population, aligning and coordinating care based on patient and family needs and connecting patients and families with appropriate resources in the Children's Center and community.
- Employ nursing processes to obtain objective and subjective data about the patient, responding to patient calls and MyChart messages requiring triage of urgent symptoms, and providing patient education for acute and chronic medical conditions.

To be successful in this role, you will need:
- Completion of an accredited RN nursing program; BSN preferred
- Minimum one year of Pediatrics RN experience required; ideal candidate will have 5 years' RN experience
- Preferred experience in case management, discharge planning or utilization review
- Valid Maryland Nursing License required
- Accredited Case Manager - Registered Nurse (ACM - RN) / Certified Case Manager (CCM) preferred
