Patient Care Manager and Field RN

We are seeking a highly skilled and compassionate Patient Care Manager and Field RN to join our team at HomeCall, a part of LHC Group. As a Patient Care Manager, you will play a vital role in providing quality support to our patient-facing teams and ensuring that our patients receive the best possible care.
Key Responsibilities:- Supervise and coordinate clinical services to ensure high-quality patient care
- Provide and direct provisions of nursing care to patients in their homes as prescribed by the physician
- Ensure patient needs are continually assessed and care rendered is individualized to patient needs
- Review assessments and plans of care daily and consult with clinicians as needed
- Current RN licensure in the state of practice
- Current CPR certification
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
