Primary Care Psychiatric Nurse Practitioner PMHNP

We are seeking a highly skilled and compassionate Primary Care Psychiatric Nurse Practitioner PMHNP to join our team at Southwest Care Center. As a key member of our healthcare team, you will provide high-quality patient care, working collaboratively with our multidisciplinary team to deliver exceptional patient outcomes.
Job Summary:The Primary Care Psychiatric Nurse Practitioner PMHNP will be responsible for providing comprehensive psychiatric evaluations, developing treatment plans, and prescribing medications as needed. You will work closely with patients, families, and healthcare providers to ensure seamless care coordination and optimal patient outcomes.
Responsibilities:- Perform comprehensive psychiatric evaluations, including patient mental status, current and past histories, and medical and social history.
- Develop and implement treatment plans, including medication management and therapy.
- Collaborate with patients, families, and healthcare providers to ensure seamless care coordination and optimal patient outcomes.
- Prescribe medications as needed, ensuring patient safety and adherence to practice guidelines.
- Maintain accurate and up-to-date patient records, adhering to governing agency and SCC policies.
- Educate patients and families about diagnosis, treatment, and medication.
- Manage psychiatric emergencies, working collaboratively with the team to ensure appropriate treatment.
- Assist in coordinating patient care referrals to community agencies, ensuring continuity of care.
- Certified Psychiatric Mental Health Nurse Practitioner with 3+ years of experience in behavioral health or psychiatric treatment environment.
- Bi-lingual in English and Spanish highly preferred.
- Current Controlled Substance Registration Certificate from U.S. Drug Enforcement Administration and from the State of New Mexico.
- Current nurse practitioner license awarded from the State of New Mexico.
- Basic Life Support (BLS) certification.
- Valid NM driver's license and reliable personal transportation.
