Amergis Staffing is seeking an experienced Medical Social Worker with a passion for delivering compassionate care to individuals in need. This Hospice position in Tucson, AZ offers a unique opportunity to make a meaningful difference in the lives of patients and their families.



Job Summary

The ideal candidate will possess a Master's degree in Social Work and have at least one year of experience in a healthcare setting. As a Medical Social Worker, you will collaborate with the multidisciplinary team to develop and implement care plans that address the social and emotional needs of patients. Your expertise will be instrumental in ensuring that patients receive the support and resources necessary to achieve optimal health outcomes.



Responsibilities


  1. Develop and implement care plans that address the social and emotional needs of patients
  2. Collaborate with the multidisciplinary team to ensure comprehensive care
  3. Provide medically necessary social work intervention to patients with social or emotional problems
  4. Assess patient psychosocial needs and plan interventions to meet established patient goals
  5. Facilitate patient referrals to community agencies when appropriate
  6. Provide financial counseling to achieve treatment goals
  7. Assist patients and families in planning for present and future psychosocial needs
  8. Adhere to company policies and procedures
  9. Report patient progress and changes in patient status to the multidisciplinary team
  10. Function as a resource person for the healthcare team


Requirements


  • Baccalaureate degree in Social Work (BSW) or Master's degree from a school of social work accredited by the Council on Social Work Education
  • Minimum of one year of social work experience in a healthcare environment
  • Complies with all relevant professional standards of practice
  • Participation and completion of Amergis' Competency program when applicable
  • Current CPR if applicable
  • TB questionnaire, PPD or chest x-ray if applicable
  • Current Health certificate (per contract or state regulation)
  • Must meet all federal, state and local requirements
  • Successful completion of new hire training as applicable to job site
  • Understand patient confidentiality and HIPAA requirements
  • Ability to effectively elicit and provide information to and from appropriate individuals
  • Computer proficiency required
  • Must be at least 18 years of age
