Key Responsibilities- Conducts comprehensive assessments to establish individualized care plans focused on client-centered outcomes.
- Regularly performs follow-up assessments on assigned cases to ensure ongoing appropriateness of care.
- Prepares and submits quarterly reports to management regarding the status of client care and reassessments.
- Creates, updates, and finalizes care plans and necessary documentation, ensuring client and caregiver input is incorporated and obtaining necessary approvals.
Must possess a valid Registered Nurse license in Michigan, with a minimum of three years of relevant experience or a combination of education and case management expertise.
Client EngagementMaintains consistent communication with clients, caregivers, healthcare providers, and other stakeholders involved in the care plan through home visits and telephonic interactions.
Education and Support- Provides education to clients, caregivers, and service providers about their respective roles and responsibilities, ensuring that services align with the established care plan.
- Conducts necessary assessments as required by care protocols.
- Bilingual candidates are strongly preferred.
This position typically follows a five-day work schedule, requiring a minimum of three days in the office, with the option to work remotely on other days, while remaining flexible to report to the office as needed for business continuity.
