Job Summary

We are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessment Specialist. As a key member of our clinical team, you will conduct comprehensive health assessments in the comfort of patients' homes, providing critical insights into their overall health and well-being.

Key Responsibilities
  • Conduct thorough adult and geriatric assessments, medical history reviews, and diagnosis and treatment planning in patients' homes
  • Collaborate with healthcare teams, including physicians, case managers, and social workers, to ensure comprehensive care
  • Provide education and support to patients and their families, promoting healthy behaviors and disease management
  • Document all assessments and care plans accurately and thoroughly, adhering to Matrix Medical Network guidelines
Requirements
  • Master's degree in Nursing or equivalent experience and licensure
  • Current RN and NP licensure, with prescription authority or ability to obtain
  • Board certification in a Matrix-approved specialty
  • 1+ year of experience as a Nurse Practitioner, with strong computer skills and familiarity with electronic health records
  • Excellent communication and interpersonal skills, with ability to work independently and as part of a team
