Collaboration:
The incumbent will work closely with healthcare professionals, sharing the responsibility for patient medication therapy outcomes.
Key Responsibilities:
Responsibilities may include:
- Ensuring the safety and efficacy of the medication use process.
- Processing and dispensing medications according to prescriptions.
- Monitoring medication therapy for effectiveness and safety.
- Ensuring compliance with regulatory standards.
- Overseeing pharmacist extenders and maintaining medication security.
- Protecting patient confidentiality and information.
Support and Continuity:
Additionally, the role requires providing drug information support to medical staff, ensuring seamless continuity of care, and identifying potential challenges in common scenarios.
Qualifications:
To be considered for this position, candidates must possess:
- A current Indiana pharmacist license or eligibility for licensure.
- Successful completion of an ASHP accredited PGY1 and/or PGY2 pharmacy residency may be necessary.
