Job Summary

We are seeking a highly skilled and dedicated Registered Nurse (RN) to join our team at Avera@Home Yankton. As a RN, you will be responsible for providing patient-centered care and services, including case management, education, and coordination of care.

Key Responsibilities

  • Develop and implement patient care plans in collaboration with the physician and interdisciplinary team.
  • Provide education and support to patients and their families to promote health and well-being.
  • Coordinate care and services to ensure seamless transitions and optimal patient outcomes.
  • Collaborate with the interdisciplinary team to ensure comprehensive and coordinated care.
  • Participate in quality improvement initiatives to enhance patient care and services.

Requirements

  • Graduate from a CCNE or NLN accredited nursing program is strongly preferred.
  • Graduate from an approved nursing program is acceptable.
  • Bachelor's degree in nursing preferred.
  • Minimum 2 years related experience and/or training; or equivalent combination of education and experience preferred.
  • Licensure as a Registered Nurse (RN) in South Dakota and/or meeting the requirements of Multi-state licensure is required.
  • Valid driver's license and insurability by the Avera Health automobile liability insurance carrier.
