Supervisory Nurse

About the Role:

We are seeking a skilled and compassionate Supervisory Visits Nurse to join our team at Addus HomeCare. As a Supervisory Visits Nurse, you will play a vital role in enabling patients to stay independent while remaining in their own homes.

Key Responsibilities:

• Perform routine supervisory visits to evaluate the plan of care and supervise delivery of excellent client service.
• Present educational in-service trainings to ensure high-quality care.
• Communicate effectively with Case Managers and other members of the care team.
• Collaborate with patients, families, and healthcare professionals to develop and implement personalized care plans.

Requirements:

• Graduation from an accredited school of nursing and current LPN or RN licensure.
• Ability to travel locally in your own vehicle and possess a valid driver's license and appropriate automobile insurance coverage.
