About the Role

We are seeking a dedicated and compassionate Pediatric Home Health Nurse to join our team at Thrive Skilled Pediatric Care LLC. As a Pediatric Home Health Nurse, you will provide high-quality clinical care to medically fragile children in their homes, enabling them to grow and thrive to their full potential.

Responsibilities
  • Provide skilled nursing care to pediatric patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for enhancement.
  • Document patient progress, including medical history, treatment plans, and outcomes, in accordance with company policies and regulatory requirements.
Requirements
  • RN/LPN with a current valid license to practice as issued by the State Board of Nursing.
  • Current CPR certification.
