LPN Medical Coordinator

The Clear View School is seeking a skilled LPN Medical Coordinator to join our team. As a Medical Coordinator, you will play a vital role in ensuring the health and well-being of our residents.
Responsibilities:- Prepare clinical consultation forms for medical appointments and maintain scheduling of medical appointments, medication records, and ensure RN/MD treatment orders are implemented.
- In collaboration with the RN and Residence Manager, ensure sufficient staff are available for each appointment.
- Review all consults immediately upon return to the facility and ensure that all medical appointments are up to date.
- Develop a weekly/monthly medical appointment calendar and maintain and obtain AMAP certification.
- Ensure that all prescriptions are submitted to the pharmacy and that medications are counted and recorded correctly.
- Process medication/treatment orders in house and with the pharmacy and check the Medication Cart/Room weekly to ensure sufficient medications.
- Reorder medication when the supply is insufficient and review that the pharmacy receives all original prescriptions.
- Perform daily MAR checks and review and log all control substances to ensure compliance and validate count.
- Ensure the Medication Room/Cart is clean and orderly and that all supplies are sufficient.
- Assist the RN in the maintenance of individuals' medical binders and assist in the completion of forms, infection control, medical appointments, nursing report, and resident report.
- Ensure pharmacy review is scheduled and completed quarterly and review/monitor Bowel Movement charts daily.
- Attend Clinical Team meetings, ISP meetings, Annual and Semi-Annual meetings, and organize consent for the treatment process.
- Graduate of an accredited Licensed Practical Nurse (LPN) program and current NYS Nursing License.
